Scholarship Enrollment and Payment Request (EPR) Form & Supporting Documents
In order for SCHEV to determine a student’s eligibility for the GUV Scholarship, students must complete Step 2 of the application by doing the following at least 2 weeks before the start of the term they want to begin receiving payment:
- Complete and submit the online GUV Scholarship Enrollment and Payment Request (EPR) Form.
Only students who participated in GEAR UP Virginia between 2014 and 2021 can apply for the scholarship.
- Email the supporting documents listed below to firstname.lastname@example.org (see note below). PDFs, screenshots and pictures of documents are acceptable. Documents must include the student and college name.
- Your college acceptance letter or class schedule, and
- Your financial aid award notification/offer showing the names and amounts of your financial aid awards. You must successfully submit a FAFSA to receive a financial aid award notification/offer from your college.
NOTE: Before emailing your documents, please blackout any sensitive information - such as student ID number or social security number - included in your documents. If you would prefer to send the documents through a secure, encrypted email, please send a request for access to email@example.com.
